Helping health and care systems sustain the changes that work

Future Care Capital (FCC) and Social Finance have formed a strategic partnership to help health and care organisations design, fund, implement and sustain meaningful change.

The partnership brings together Social Finance's expertise in system transformation, outcomes-based contracting, finance and investment with FCC's expertise in health and care innovation adoption and system change.

It has two complementary elements: joint investment in developing the Orchestrator, a programme helping local systems make a sustained shift towards prevention, and a wider collaboration to help health and care organisations tackle complex challenges and deliver sustainable transformation.

What’s the challenge?

Health and care organisations face complex challenges that often require changes across services, organisations, funding arrangements and ways of working.

Even where there is a clear understanding of what needs to change, turning that ambition into practical, sustainable improvements can be difficult.

Promising innovations and new models of care can struggle to move beyond individual projects or pilots. Without the right capacity, partnerships, governance and funding arrangements, improvements can be difficult to implement, embed and sustain over the longer term.

How can FCC and Social Finance help?

Depending on your organisation's challenges and priorities, FCC and Social Finance can help you:

  • Understand complex system challenges, identify opportunities for improvement and assess what needs to change.

  • Design new models of care and develop the financial arrangements needed to support them, including outcomes-based contracting where appropriate

  • Bring together organisations, partners and stakeholders across health, social care, local government, the voluntary sector and industry to build shared understanding and commitment.

  • Support the adoption and implementation of innovation and new models of care, drawing on FCC's Innovation Onboarding framework to assess readiness, plan implementation and build the capabilities needed for sustained delivery.

  • Identify barriers to change and establish the governance, delivery arrangements and practical plans needed to embed improvements across organisations and services.

  • Evaluate outcomes and impact, generating evidence of what has changed and informing further improvement and potential scaling.

Organisations often need several of these capabilities to address complex challenges. By bringing together complementary expertise in system transformation, financing, innovation adoption, implementation and evaluation, the partnership can help organisations move from understanding a problem through to delivering and sustaining meaningful change.

What’s the Orchestrator?

The Orchestrator is a programme being developed jointly by Social Finance and Future Care Capital (FCC) to help local systems make a sustained shift towards prevention.

It is designed for health and care systems, local authorities, regional bodies and their public and voluntary sector partners seeking to move beyond short-term initiatives and embed lasting change.

The Orchestrator works in three complementary ways:

  • It helps local systems understand and reshape how funding flows, aligning resources with agreed outcomes and supporting a shift from costly crisis responses towards earlier, preventative support that can be sustained through core budgets.

  • It starts with the needs of the system. It helps local partners understand their challenges, identify opportunities for improvement, assess existing capabilities and agree where additional expertise or support is needed.

  • It supports implementation and sustainability. It helps organisations identify barriers, align partners and establish the governance, capabilities and practical delivery arrangements needed to implement, embed and sustain change over the longer term.

For example, a system could use the Orchestrator to invest in more support for older people with multiple health conditions to live well at home, helping to avoid repeated A&E admissions and long hospital stays.

The Orchestrator supports the three shifts in the NHS 10 Year Health Plan: from hospital to community, from analogue to digital, and from treatment to prevention.

Why FCC and Social Finance?

Future Care Capital (FCC) is an independent charity helping health and care organisations tackle challenges, realise opportunities and create sustained impact. It brings expertise in health and care innovation adoption and system change, alongside practical experience in pathway redesign, implementation and evaluation.

FCC's Innovation Onboarding framework provides a structured approach to helping organisations assess readiness, plan implementation and embed innovation and new models of care into practice.

As an independent charity, FCC is not tied to any provider, technology or source of capital, enabling it to bring together partners and expertise around the needs of the organisations and communities it supports.

Social Finance is a not-for-profit organisation with 20 years of experience in financing, investment structures and system transformation, including outcomes-based contracting. In 2010, it launched the world's first social impact bond. Its long-running partnership with Macmillan Cancer Support has demonstrated how outcomes-based contracting can support sustainable new models of care.

Together, FCC and Social Finance bring complementary expertise across system transformation, financing, innovation adoption, implementation and evaluation. This enables the partnership to support organisations not only in developing solutions to complex challenges, but also in putting the arrangements in place to deliver, evidence and sustain meaningful change.
