Across England, Integrated Neighbourhood Teams (INTs) are being introduced as a way to strengthen collaboration between primary care, community health, and voluntary services. But what happens when one of those teams takes a bold step forward, blending AI technology with bottom-up leadership to reimagine proactive care?
Future Care Capital collaborated with Arbennek Primary Care Network (PCN) in Cornwall to assess this. Using Brave AI to identify patients at risk of unplanned hospital admission, the Arbennek INT brought together NHS, social care, and third-sector professionals to deliver person-centred, preventative care.
Our independent evaluation, led by Dr Melanie Fraser with support from Professor Andy Jones, explores how the team formed, what made it work, and what other areas can learn.
It draws on rich qualitative research and applies Tuckman’s stages of team development — Forming, Storming, Norming, Performing, Adjourning — to understand what supports real, sustained integration.
