Proving impact to drive adoption, scale and investment in innovation in health and care
The NHS and the wider health and care sector are in a state of flux, undergoing the most significant redesign and reorganisation in a generation.
Organisations must prove the real-world impact of their innovations to secure adoption, funding and scale.
Health and care leaders know they must innovate. But funders, decision-makers, and commissioners demand robust, independent evidence before committing to a large-scale rollout or investment.
Yet evaluation methods often fall short.
Static reports don’t drive action – they fail to provide a compelling reason to adopt, scale or fund innovation.
Worse, many NHS and care organisations lack the internal capability to measure ongoing impact, leaving them without the tools to build a lasting case for change.
At Future Care Capital (FCC), we don’t just assess impact – we help you amplify and embed it.
Our evaluations go beyond one-off reports, building long-term capability so organisations don’t just receive insights – they are empowered to act on them.
We work alongside your team, demystifying the evaluation process, embedding impact measurement into your operations and equipping you with the tools to secure funding and drive change.

This independent evaluation explores the creation and impact of an Integrated Neighbourhood Team (INT) within Arbennek PCN in Cornwall. Using Brave AI to identify patients at risk of hospital admission, the team brought together health, care, and community professionals to deliver more coordinated, proactive support.
The report applies Tuckman’s stages of group development to analyse what worked, what didn’t, and what others can learn. It offers practical recommendations for sustaining local innovation, supporting leadership, and embedding relational, bottom-up approaches into system design.
